Hours of Operation: The PACT Team is a 24 hour/7 days a week operation with on call crisis workers available for consumers after open office hours of 7:00 A.M. to 5:00 P.M., please leave a message and the on call staff will return your calls.
Ages Served: Persons age 18 and older
Description of Services Provided: Program for Assertive Community Treatment (PACT) is a team approach designed to provide comprehensive community based psychiatric treatment, rehabilitation and support to persons with serious and persistent mental illnesses. The multi-disciplinary team consisting of case managers; therapists; peer support; psychiatric medical management services; nursing; employment and housing services; substance abuse services; function interchangeably with an attitude of "whatever it takes" to assist the client to live successfully in the community. PACT services are available 24 hours a day, 7 days a week, 365 days year. Client outreach, relationship building, individualization of services and long term continuity of care are highly emphasized. The team has the ability to have multiple daily contacts with clients.
